Am I Eligible?

You may be eligible if you hold:
  • A DVA Gold Card
  • A DVA White Card, with an accepted service-related condition requiring nursing care
  • A GP or specialist referral identifying a clinical nursing need
War widows and widowers may also qualify. Not sure of your status? We can help you check.

How Do I Get Started?

To get started with our support
  • Speak with your GP and ask for a Community Nursing referral
  • Contact us and request UnitingCare Community Services as your care coordinator
  • We liaise with the contracted nursing provider on your behalf
  • Care begins once your referral and care plan are confirmed

What Other Support Can Veterans Access?

Alongside nursing coordination, we help arrange a broader range of everyday and personal support for veterans, delivered according to your assessed needs and approved care plan:
  • Light domestic cleaning and dishwashing and tidying
  • Meal preparation
  • Personal hygiene support and dressing assistance
  • Mobility support and toileting support
  • Wellness and safety checks and short-term monitoring
  • Including 24/7 care where arranged
  • Emotional support and routine building
  • Phone support and DVA coordination support
These services are arranged as an associated service provider working in partnership with registered aged care and DVA-approved providers, according to your assessed needs, funding and approved care plan.
